What is an integrated or inter-professional healthcare team?

An integrated or inter-professional healthcare team is a diverse group of providers such as physicians, mental health clinicians, dietitian’s and other professionals who work together to support a shared patient. The aim of this model, for-patient-care, is to provide a holistic approach to understanding and supporting a patient's wellness; each practitioner provides their unique perspective and expertise within the team. This is a person-centred approach; therefore, the patient is actively engaged in decisions regarding their health care.

What can I expect from a virtual appointment?

I encourage patients to set themselves up in a private and comfortable space. If possible, please bring with you something to hold, for example, a small stone or a trinket that is meaningful to you. Please feel free to draw, doodle or play with play-dough during your counselling session. I do not require that your camera is turned on. Please make sure that you have a solid internet connection. At the start of any virtual appointment, I ask patients to provide me with their exact location and contact number in case our connection is lost and I have a safety concern for the patient. It is my ethical responsibility to keep this information confidential unless I am concerned for the safety of my client or of another person (more information on my ethical responsibilities is within your confidentiality agreement document).

What can I expect from an in-person appointment?

In-person appointments are available at Westward Medical in a private office. I provide patients with a comfortable space, drawing and other materials to use during the counselling session. I encourage patients to bring a water bottle with them. Please note, that you may be asked to wait in the clinic waiting room before your appointment and that you are required to wear a mask in this public space.
